3) Date of birth (with year, month and day)
4) Country team
5) Whether you'll be taking part in Parent-Child Doubles (If "Yes", also provide parent's name and relationship)
6) Whether you'll be taking part in Age-Divisional Doubles (Please note that Doubles partner must be also registered for this competition and must be at the same physical location during the competition)
